Hip and pelvic pain is complex — the region is shared by the lumbar spine, the hip joint, and a network of tendons and bursae. Pinpointing the source is the first step, and it's often not where you expect.
Common causes
Hip impingement (FAI)
Bony contact within the hip joint during movement. Common in athletes and active adults. Causes deep groin pain with hip flexion.
Gluteal tendinopathy
Overload or compression of the gluteal tendons at the greater trochanter. Classic "side hip" pain, often worse with sitting cross-legged.
SIJ dysfunction
The sacroiliac joint is a frequent driver of low back, gluteal, and groin pain — especially after pregnancy or asymmetrical loading.
Hip OA
Gradual joint degeneration causing stiffness and groin pain. Manual therapy and exercise significantly improve function even in moderate OA.

Is my pain from the hip or the back?
Both are possible — and often co-exist. We assess the lumbar spine, SIJ, and hip joint as a system to find the primary driver.
Can you treat hip pain during pregnancy?
Yes. Pelvic girdle pain and SIJ dysfunction are common in pregnancy and respond well to gentle osteopathic care.
Do I need a hip replacement?
Only imaging and specialist review can answer that. Many people with significant hip OA continue to function well with conservative management.
