Sciatica during pregnancy: what's causing it and how osteopathy can help

Pregnancy-related sciatica is one of the most common reasons expecting mums seek osteopathic care. The sharp, shooting, or burning pain that radiates from the lower back through the buttock and down the leg can range from a mild nuisance to something that disrupts sleep, walking, and daily life. The good news is that there's a lot that can be done to ease it safely throughout pregnancy.

Why does pregnancy cause sciatica?

Several changes happening in your body during pregnancy can contribute to sciatic nerve irritation. As your baby grows, your centre of gravity shifts forward, changing the curve of your lower back and altering how load is distributed through your spine and pelvis. The hormone relaxin loosens ligaments throughout the body to prepare for birth, which can make joints — particularly in the <a href="/conditions/sacroiliac-joint-pain-and-dysfunction">sacroiliac joints</a> of the pelvis — less stable and more prone to irritation. On top of that, postural muscles like the piriformis (a small muscle deep in the buttock that sits close to the sciatic nerve) can tighten and spasm in response to these changes, sometimes compressing or irritating the nerve directly.

Is it the same as regular sciatica?

Not always. True sciatica involves irritation of the sciatic nerve itself, often from a disc issue or nerve root compression. But many cases of "sciatica" during pregnancy are actually referred pain from the sacroiliac joints or piriformis muscle, which can feel very similar — pain down the back of the leg, sometimes into the calf or foot. The distinction matters because the underlying cause shapes the treatment approach, which is why a thorough assessment is so valuable.

What can osteopathy do about it?

Osteopathic treatment during pregnancy focuses on easing the mechanical strain that's contributing to your symptoms. This might include gentle, hands-on techniques to improve mobility in the lower back and pelvis, release tight muscles like the piriformis and glutes, and take pressure off irritated nerves. Treatment is always adapted to your stage of pregnancy — positioning, technique, and pressure are modified to keep you comfortable and safe, particularly as your pregnancy progresses. Many women find relief within just a few sessions, and ongoing care can help manage symptoms as your body continues to change.

What can you do at home?

Alongside treatment, a few simple habits can make a real difference. Sleeping on your side with a pillow between your knees helps keep your pelvis aligned. Gentle stretching for the glutes and hip rotators — like a seated figure-four stretch — can ease piriformis tightness. Avoiding long periods of standing or sitting in one position, and taking regular short walks, can also help keep things moving and reduce stiffness.
